Thank you for your interest in our 14th Annual Holiday Toy Drive! We are so excited to help you bring holiday magic to your family this season.

Please only complete this form if you are the legal guardian of the child you are requesting for. Proper ID will be required to receive toys and toys cannot be requested for extended family or friends.
Donation requests can be submitted for children under the age of 18 in the South Florida area.

Toy pick-ups will be held in December in Jupiter, Lake Worth, and West Palm Beach. Toys will not be delivered. 
Pick-ups will be scheduled individually with a representative from the Toy Drive.

If you miss your scheduled pickup there is no guarantee an alternate day/time can be rescheduled.

Please keep in mind, toy donations are first-come, first-serve. Toys will be pre-packaged for your family and available to pick up only on the day given to you by a Little Smiles representative. 
We will try our hardest to get your child toys that that will love. Please know that toys are all donated by our generous community and will not always be exactly what was requested. 

Be sure to only submit this form ONCE. Multiple submissions can delay or possibly deny your application.
